Good Faith Estimate

Good Faith Estimate

At Trilogy Chiropractic, we believe in transparency and want you to understand your rights regarding the cost of your medical care. Under the No Surprises Act, healthcare providers must provide patients who don’t have insurance or are not using insurance with an estimate of the expected charges for medical items and services.

Your Rights:

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency chiropractic care, including related costs such as medical tests, equipment, and other associated fees.
We will provide a written Good Faith Estimate at least one business day before your scheduled service or item. You can also request a Good Faith Estimate before you schedule a service or item.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
Please save a copy or take a picture of your Good Faith Estimate for your records.
